In accordance with Texas House Bill 4224 and Texas Health & Safety Code §181.105, the following information explains how to request your health care records, contact the appropriate licensing authority, and file a consumer complaint.

Requesting Your Health Care Records

Current and former clients of Open Skies Counseling may request access to their health care records.

To begin a records request, please email:

admin@openskiescounselingtx.com

Please include your name, a way to contact you, and a brief statement indicating that you would like to request your records. Do not include sensitive clinical information in your initial email.

You may be asked to complete a written authorization or provide information to verify your identity before records are released.

Requests will be processed in accordance with applicable Texas and federal privacy laws. Records will be provided through an appropriate, secure delivery method.

Filing a Consumer Complaint

Consumers may file a complaint with the Texas Office of the Attorney General's Consumer Protection Division.

Notice to Clients

The Texas Behavioral Health Executive Council investigates and prosecutes professional misconduct committed by marriage and family therapists, professional counselors, psychologists, psychological associates, social workers, and licensed specialists in school psychology.

Although not every complaint against or dispute with a licensee involves professional misconduct, the Executive Council will provide you with information about how to file a complaint.
